I see many patients who come to me unhappy with their nasolabial folds (lines from their nose to their mouth) who want filler. However it is not necessarily that straightforward. The nasolabial folds form as a result of ageing; we lose volume in our cheeks, because of the fat pads shrinking and the bone resorbing. But we actually gain volume in the nasolabial folds as firstly the fat pad in that area increases in size over time but secondly gravity pulls down the tissues from above. If we just add filler to this fold without addressing the underlying cause then we create a mismatch and add to the area that has already gained volume but do nothing to address the area that has lost volume. This disharmony in treating the face is what can cause distortion and for people to look a bit weird or cat like. It goes without saying that this isn’t a look I want for my patients! Therefore it’s very important to treat the cause of ageing not just the signs. By treating the cheeks with filler you firstly create a more youthful look but secondly there are techniques used which can act as scaffolding for the face and literally lift up some of the tissues. This can indirectly help the nasolabial folds before any filler has gone near it. There will be occasions where I will still treat the nasolabial folds directly with filler but by treating the face harmoniously beforehand, less filler is required in the fold and most importantly the overall look is natural.

Aftercare

After treatment, I advise my patients not to do anything that will make them hot or flushed (red in the face) otherwise there is a risk of the Botox spreading to a muscle we are not intending to treat. Examples of this would be exercise, saunas, sunbeds (don’t use sunbeds ever anyway!), lots of alcohol or facials. After 24 hours you can do any of the above as the Botox won’t move after this.

What Are They?
Botulinum toxin is injected carefully into the muscle, which temporarily blocks the signal from the nerve, leaving the muscle in a relaxed state. There are many different brands of botulinum toxin but I use Botox, made by Allergan.
What Are The Benefits?
The main benefit of Botox injections is a reduction in wrinkles, and softening of established lines, but Botox only works on areas of movement. However, Botox can also be used in the masseter muscles to relieve jaw clenching or teeth grinding which alleviates tension and headaches, and minimises dental damage. Interestingly, Botox can also be used to stop sweating so can be injected under the armpits or into palms or soles of feet, with excellent effect.
Are They Painful?
Botox injections sting a tiny bit but I would rate it at 1-2/10 on the pain scale. In my opinion, pain should absolutely not be a barrier to botox treatment for anyone. If necessary, numbing cream can be applied beforehand.
How Long Is Each Session?
My botox appointments are 20 minutes, but once your dose is established it might not even take that long.
